Wheel/tire came off trailer

This morning local news said 1 killed when wheel/tire came off a passing trailer. It hit oncoming vehicle. That vehicle drove off the road and driver was killed.
It did not mention RV trailer but on that highway it likely was a RV trailer.
No reason given why, just a reminder to check those lug nuts and wheel bearings.

Here's an update:
On Monday, October 4, at 3:32 p.m., Idaho State Police investigated a four-vehicle crash that occurred on State Highway 55 near milepost 119, just north of Cascade.

A 2019 Dodge Ram pickup pulling a utility trailer, driven by a 53-year-old male from Ogden, UT, was driving northbound on State Highway 55. One of the trailer tires broke off the trailer and entered the southbound lane. A 2016 Ford F350 pulling an enclosed trailer, driven by a 32-year-old male from Caldwell, was driving southbound on State Highway 55. The Ford struck the tire and crossed into the northbound lane where it struck a 2016 Honda Accord, driven by a 58-year-old female from Boise, head on. The Ford and Honda both went down an embankment. The Ford's trailer broke loose and continued southbound on State Highway 55 where it struck a 2013 Nissan Versa, driven by a 55-year-old male from Donnelly.

The driver of the Honda was wearing a seatbelt. She succumbed to her injuries at the scene of the crash. Notification has been made to next of kin.

The driver of the Ford and the Nissan were transported to a local hospital. The roadway was blocked for approximately 4 hours. Idaho State Police was assisted on scene by the Valley County Sheriff's Office and the Idaho Transportation Department. The incident remains under investigation by Idaho State Police.

Wow, what a crazy set of events in that crash. So sad that it resulted in a fatality.

Jim, thanks for following up with correct details. I was going to look it up and post the real facts been busy all day.
I have been up/down that highway, it's mostly steep, narrow and curvy with a river next to or below it.

Ken, I looked for a more detailed article, but none was to be found. When I read this one I had to go back and read it again because it was a little confusing with so many vehicles involved.

My new axles are in and will be installed Monday morning.... Why?

Two years ago we returned from a 2 month trip to the southwest. Through Colorado, the roads were the worst I think I've ever encountered (I-70 through the mountains)..... A couple weeks after we returned, we headed downstate to a family campout near Waterford, MI. As we turned into the campground and passed the "guard shack", there is a small speed bump. We were travelling around 3 or 4 MPH as we crossed that speed bump. As the trailer wheels climbed the bump, I was watching them in the side mirror. I noticed the trailer fender "bulge out" and immediately slammed on brakes. The roadside rear wheel/tire/hub had completely separated from the spindle.

We had just towed at 65-70MPH on I-75/I-96 in extremely heavy traffic. We had stopped at the last rest area on I-75 (about 20 miles from the campground) and my usual checks with a digital thermometer were normal.

It's extremely un-nerving to think what could have happened had that axle "come unglued" just 10 or 15 miles sooner than it did.....

So, after replacing the spindle on my 5200 pound "derated to 4400 pound" axles, and just not trusting it any longer, I ordered two 6000 pound Dexter axles. Monday afternoon, everything below the spring hangers will be "not Lippert"....

None of us know "when or where" it'll happen next.

Good point, John.
Loose lug nuts is not the only cause.

I am guessing here, but for some reason, a wheel bearing failed, one of the roller bearings got twisted in the cage and pushed against the bearing retaining washer hard enough to gouge it and cut the cotter pin. When I got stopped, the wheel/hub was still "partly on the spindle" but the bearing washer and nut were in the hub cover with the cotter pin still on one side of the nut. The cotter pin had sheared where it went into and came out of the spindle, so it was "literally cut in two places, allowing the nut to back off the spindle"....

Looking back, I should have probably inspected the bearings after the trip to Las Vegas. I had repacked them before that trip, so better than 8000 miles with no problems, but maybe the "rough interstate towing" did more than I suspected....

At any rate, I'm no longer towing "with axles that barely meet the trailer GVWR" or on "Lippert/china components" bought at the lowest bid by Keystone..... Can't really complain though, after all, they are 6 years old (with annual maintenance performed by me)....
