Could it be?

My 2020 Montana has a vent pipe under island that comes to just below sink. If you’re Grey tank for that sink is full I’m curious if the water would spill out that vents cap to your island floor.. no obvious leak in piping so tank water splashed out???

That vent should be higher than the sink drains so in theory it should back up into the sink before leaking from the vent, but as I said IN THEORY.

That device is an air admittance valve which is used instead of a vent through the roof when a vent can not be installed. If it is working properly, it should keep waste from exiting the drain, but just a little gap from debris can cause it to leak. We've had a Studor (air admittance) valve under our kitchen sink for 17+ years without any issue; in the past the sink has backed up but there was no spillage from the valve.

Studor is the only vent valve of this sort I would use. We had these under the bathroom sinks and the cheapo vent valves allow stink from the pee trap to waft out. The Studor vents ARE the fix. The cheapos cost about $7 at a box store and Studor cost about $30 if memory serves.
