They replaced the tire and rim.
1) I keep the tires at 80psi cold and had only been on the road for 15 minutes under 60mph. Could I have possibly picked up something on the road or are these tires junk? If so, what is a better tire for a single axle. We don't go more than 60 miles one way to camp.
2) I'm leery of going out again until I figure out what happened. This is a single axle. If I had a tandem, I figure I would have at least saved the rim?
Thanks,

The tires typically provided from the factory are low bid specials....many complaints of early life blowouts. Google 'China Bombs'. Don't know if Loadstar falls into this category, but at $150 for a tire
and rim, suspect the quality may not be all that great.
Do you see any indication of potential other problems? Rub marks in the wheel well area? Bent axles? (Unless overloaded, they should have a slight curve - high point in the center - to them). Unusual wear on the other tires?
I would suggest swapping out all 4 tires for a more reputable ST tire replacement like Goodyear Endurance, Carlisle HD or Sailuns.
