Exclamation Discovered unhooked up brakes on middle axle from factory

I purchased a new 2010 Keystone Raptor VELOCITY 3812TS Toy-hauler about
two years ago.

I run a retail business and have my own garage, with three full-time mechanics who maintain my trucks, trailers and other vehicles.

Recently while replacing tires on the Raptor, and also doing preventive
maintenance, such as checking the wheel bearings etc. I was shocked to learn that one of my mechanics had discovered that both brakes (both right and left)
on the middle axle had never been connected from the factory.

Has anyone else had experience of this sort?
