You don't say what rig you have, but we had a caster come off years ago. It ended up the caster was OK, but the cleat that it was attached to had cracked which caused the caster to pivot up. I had to remove the mattress and both the plywood panel that supports the mattress plus the panel below that supports the small storage area below the mattress. Then I had access to the caster and was able to make repairs. The photo shows what we have below the bed, and the failed caster was on the side I was standing. On the far side, you can see the cleat along the bottom of the bed side and that is where the caster is mounted to, but it's not visible due to the metal slide bracket being in the way.
