I normally use a couple pieces of 3/8 plywood when i get on the roof to help spread the load some, and they are light enough to get up the ladder

With slide out, pull up the floor covering/rug at the transition. You will see the hard poly material used to raise and guide the slide when coming in. Maybe its out of whack or has an edge catching

Replacing the wood is the right way.....sorry....sux i know. However there are leveling products out there used on plywood floors to bring things closer to level say when putting vinyl floors in....
